This application form is for the Toloa Substantial Provider Fund.

The Toloa Substantial Provider Fund supports proven and experienced providers deliver STEAM programmes that activate meaningful STEAM engagement for Pacific communities. Programmes must directly contribute to the long term growth of Pacific STEAM. Funding is available for 1+ year(s) of programme delivery.

Before you proceed, we recommend you have the following attachments ready to upload during your application:

  • A certificate of organisation status.

  • A detailed project plan.

  • A detailed budget expenditure.

  • A detailed timeline of programme delivery.

  • Your business strategy and organisational system and processes.
  • Provide 5 letters of support from partners, collaborators and participants.

  • Provide evidence of 3 or more years of successful programme delivery by your organisation.

  • Independent audited financial accounts for the most recent financial year.

  • Previous Toloa fund recipients are required to attach independent audited reports for the years of which they were previously funded.

  • Any other relevant support material to strengthen your application.

*Applications for multi-year funding require an operational site visit prior to finalising funding.

I declare and agree that:

  1. I am authorised to act on behalf of this organisation (if applicable).
  2. The information provided in this application form is true and correct to the best of my knowledge.
  3. If successful all requirements outlined in this form will need to be fulfilled.
  4. If successful I will need to supply the Ministry for Pacific Peoples with a scanned copy or screenshot of the bank account number funds will need to be deposited into.
  5. The fund will be used solely for the purpose of the project outlined in this application.
  6. The fund will not be used for the purchasing of resources for personal use. 
  7. The project in this application will be delivered no later than June 2022. 
  8. At the completion of the project, the project coordinator will provide an accountability report on the Project. The report will outline the successes, any challenges faced, and outcomes achieved. This is due within four weeks of completing the project.
  9. I will advise the Ministry if for any reason it I am unable to deliver all or any part of the application or need to make changes.
